Form Purpose
Form 1 Application to Make and Register a Firearm
This form is used by individuals to manufacture a firearm.  There is a $200 tax paid for manufacturing, even for an AOW.  The form must be accompanied by photographs, two fingerprint cards, and the law enforcement signature on the back. 

NOTE:  ATF recently changed the Form 1. The old forms are obsolete!! This is the current form (12/31/2001).
 
Form 2 Notice of Firearms Manufactured or Imported
This form is used by a manufacturer or importer to notify ATF of your activities.  You must file a Form 2 within 24 hours of manufacturing or importing a firearm.
Form 3 Application for Tax-Exempt Transfer of Firearm and Registration to Special (Occupational) Taxpayer
This form is used to transfer items tax-free between Title II dealers.
Form 4 Application for Tax-Paid Transfer and Registration of Firearm
This is the form that most of us use -- it is the form that you will use to transfer an item from a dealer in your state.  You can also use this form to transfer an item from an individual in your state.

NOTE:  ATF recently changed the Form 4. The old forms are obsolete!! This is the current form (12/31/2001).
 
Form 5 Application for Tax-Exempt Transfer and Registration of Firearm
This form is used to transfer items to and from a government agency and for transfers to a lawful heir when you kick the bucket.  It is also used to transfer an item back to the manufacturer for repair.
Form 6 Application and Permit for Importation of Firearms, Ammunition and Implements of War
Just like it sounds

NOTE:  ATF recently changed the Form 6.  I'm not sure if you can use the old form... In any event, here is the new form.

Form 7 Application for License
This is the application for the FFL.  If you fill out the first two pages and print this document, it will also print out pages 3 and 4 with the same data -- the copy you need to provide to LE.  IOW, don't worry about trying to complete pages 3&4, they'll already be done.  By the way, you must use 8.5x14" paper and you should copy it on yellow paper.  I've heard rumors that ATF will accept this on plain paper, but just run down to Kinko's and copy the dang thing on yellow paper for piece of mind.
Form 7CR Application for License (Collector of Curios and Relics)
This is the application for the C&R FFL. Same thing as above for the 2nd copy.  You must use 8.5x14" paper and this one should be printed on green paper.  Again, go to Kinko's and save yourself the worry that some goof will toss it in the trash because it doesn't look right. 
Form 9 Application and Permit for Permanent Exportation of Firearms
You need to send this in in quadruplicate.  And you need to include the DSP-5 from the State Department.
Form 10 Application for Registration of Firearms Acquired by Certain Governmental Entities
Don't use a Form 10 for ANY transferable gun!!! Once something goes on a Form 10, it can never come off!!! A Form 10 is essentially used for a PD to register a gun that it confiscated from somebody.

5300-11 Annual Firearms Manufacturing and Exportation Report
Federal Firearms Licensees who hold either a Type 07 (manufacture of firearms), or a Type 10 (manufacture of destructive devices) must file in compliance with 27 CFR 178.126.

5320.20 Application to Transport Interstate or to Temporarily Export Certain National Firearms Act (NFA) Firearms
Used for either a temporary visit to another state, or for a permanent move to another state.  You do not need to use a 5320.20 for an in-state move.

This one already has room for 7 items on each form.  ATF has approved these for me in the past -- but make sure it's legible before you send it in!
5330.4 Application to Register as an Importer of U.S. Munitions Import List Articles Just like it sounds.
FB-258 Blank FBI Fingerprint Card
This form is the most simple, yet will require some effort on your part in order to successfully use.  It's really a *blank* card.  The Feds told me they would not accept a computer generated card, so they must be printed on actual cards.  The benefit of this form is that you can feed it through your printer and have everything line up perfectly.  And it's cool.  Fingerprint cards used by the NFA branch of ATF must specify "DCATF0100, ATF/Firearms & Explo Div, Washington DC" in the ORI box.  The purpose of the ORI box is to indicate to whom the FBI should return the cards.  If this is blank, or worse, incorrect, your forms will never be approved as ATF will never receive your cards back from the FBI.  So... you must use a real fingerprint card with the proper ORI data in order to use this form.  You must also specify a paper size of 8"x8".  You will need to specify a custom paper size with your printer driver.  I'm using an HP LaserJet 4000 and 4500 and it works fine.  Note that some drivers do not provide for custom paper size.  You can use File/Printer Setup in the Acrobat Reader or modify your printer form the Control Panel.  If you're using one of the above printers and you can't get it to work, bounce me an email and I will try to help out.  You're on your own if you're using any other printer! Sorry.

International Import Certificate
In accordance with an agreement between the Departments of Commerce, State, and Treasury, Import Certificates issued to facilitate international cooperation in export control matters have been standardized.  Under this standardization these agencies will use the same form.  The US Department of the Treasury issues the form for article enumerated on the US Munitions Import List in connection with the issuance of a Treasury Department Import Permit.  The US Department of State issues the form in connection with foreign transfer by a US entity of US Munitions List articles.  The US Department of Commerce issues the form for all other commodities subject to an Import Certificate requirement.
